Abstract
The purpose of the Online Grade Inquiry and Submission System for CHMSC Talisay - Office of the Registrar is to establish a system which will enable to solve the problems occurring on the grade inquiry and submission system of the CHMSC - Talisay Registrar.
The proposed system aims to provide solution to the problems that was identified by the proponents during the data gathering such as slow processing of rating reports, inaccurate grades, time consuming in grade submission and requesting of grades, and loss of rating reports. Based on the stated problems, the findings of the study revealed the significant need for an Online Grade Inquiry and Submission System (OGISS).
The Proponents created the system which will serve as a solution to the problems. It will enable the teachers to submit the student's grades via online, provide site for the visibility of grades for the college students provided by the ID numbers that will serve as username in order to access the system,to avoid repetition of requesting for rating reports and minimize the manual process of both grade inquiry and submission process. The Registrar will serve as the administrator of the system. Through developing the Online Grade Inquiry and Submission System, the school specifically the office of the Registrar will be able to go with the trend of evolving technology.
The proposed system can be access via online. With the system features presented in the earlier chapters of this documentation, the proponents conclude that implementing this project is a must. This means that using the system will help their office to provide convenience and proves significance to the beneficiaries of the system. Developing the OGISS is the best solution for the company to successfully meet their business goals.
Furthermore, the proponents, conclude that implementing the OGISS to the office of the Registrar helps them to gain a competitive advantage and will be able to go with the trend of evolving technology.

Manual operation in an organization tends to slow down transactions. Thus, implementing information system is necessary. In the case of repair and maintenance office of the BSIS Department of CHMSC - Talisay, manual operation was used. The proponents used Rapid Application Development method as the approach in developing the system which helps the user to understand better the system through interactive and simultaneous revisions, which bring the system closer to what is needed. During the interviews, observations and thorough follow-up of the proponents, it was found out that there were problems encountered. The said office has a slow and inefficient delivery of services. Pulled-out properties from laboratories were not properly monitored, there was a difficulty in identifying the existing properties. Furthermore, checking of the repair request was not properly scheduled. The urgency level of repair request was difficult to identify and information about repair request status was not properly monitored. The propose Maintenance and Repair Service Management System aimed to help BSIS Department to provide a fast and efficient services through the use of new system. Provide reports for pulled-out properties from the laboratories, record all existing properties, generate and print stickers for the monitoring of property history. Forward repair request queued by date in ascending order, filter repair request according to urgency level and provide real time monitoring on the status of repair request. The system focuses only to the operation of BSIS Office for the technical services and laboratory monitoring through the system which is integrated with other systems.

"Best Care, Best Service" is what hospitals aim to provide. By automating hospital transactions, healthcare organizations can minimize the challenges in managing patient care. However, conventionally, public hospitals such as Teresita Lopez Jalandoni Provincial Hospital use manual system or traditional file system in most of their transactions like inventory, record keeping and billing. This is such a tedious work for processing and is time-consuming for both the personnel and the patient.
The study entitled "Hospital Management System" was conducted to provide fast and efficient services to its patients in regards to their hospital bill. The study will serve as a new instrument for the technological advancement that will greatly benefit the hospital, its personnel and patients.

Abstract
The Dental Clinic Record Management System is a specialized method proposed for Florence T. Balgos Dental Clinic of Bacolod City, Negros Occidental to automate the process regarding customer account information. Hence, this can help the clinic to efficiently manage their registry and records to keep track of patients' dental condition. The proponents used informative and qualitative research approach to gather relevant data from the respondents. These facts were then documented and reviewed for further development.
Moreover, the Iterative and Incremental Development Model was used to ensure the sequential completion of the said system. The proponents used PHP and MySQL to create the interface after which actual coding was settled for function on its layout. In addition, system integration and testing were done to identify problems to correct and resolve the errors and to finalize the proposed method. Definite requirements were also identified for the proper implementation. This system can assist the clinic in generating summarized monthly reports more efficiently. Additionally, more data are gathered for future study and development of the system.
